*Passport must be valid for a minimum of six months
*Visa applications must be submitted in person with the exception of residence of the District of COlombia, Kentucky, Ohio, Maryland, Virginia, and West Virginia.
To apply for a visa you must have a completed application form, a passport size photo, and proof of onward/return transportation.
If you have recently traveled to certain countries (
Angola, Benin, Bolivia, Burkina Faso, Cameroon, Colombia, Ecuador, French Guyana, Gabon, Ghana, Gambia, Guinea, Liberia, Nigeria, Peru, Sierra Leone, Sudan, Democratic Republic of Congo or Venezuela
) an inoculation card which indicates yellow fever immunization is required.
Children ages 3 months to 6 years are required to have an international polio vaccination certificate.
If you are traveling with a minor (under 18) both parents must accompany the child or you must have a notarized letter of consent signed by the non-accompanying parent(s) or guardian authorizing the consulate to issue a visa.
